I ASSUME ALL RESPONSIBILITY FOR ANY INJURIES THAT MY CHILD MIGHT SUSTAIN DURING THE CAMP. I HERBY AUTHORIZE THE STAFF OF COUGARS BASEBALL CAMP TO ACT FOR ME ACCORDING TO THEIR BEST JUDGMENT IN AN EMERGENCY REQUIRING MEDICAL ATTENTION AND I HEREBY WAIVE AND RELEASE COUGARS BASEBALL CAMP FROM ANY AND ALL LIABILITY FOR AN INJURIES INCURRED WHILE ATTENDING THE BASEBALL CAMP.
